Immunization Information

Alabama’s School Immunization Law Section 16-30-4 states each student enrolled in day care, Head Start, and public or private schools have a valid Certificate of Immunization (COI) — previously known as “blue cards” — or a Religious Exemption Form on file at the facility they attend.

School Rules Chapter 420-6-1 (1) states the COI will be on a form approved by the Alabama Department of Public Health (ADPH), which is the one printed from ImmPRINT. All required vaccines must be recorded before the COI can be submitted to the schools.

In addition to standard required and recommended vaccinations, there are new immunization requirements for the school year that may affect your child if they are:

  • Attending day cares or Head Start programs: PCV- pneumococcal conjugate vaccine
  • 10-11 years old: Tdap vaccination.
  • Required for all grades (K5-12): Varicella vaccination.

With school quickly approaching, parents can get their childrens Back-to-School Immunizations by visiting a MCHD Health Center before the last-minute rush begins. All required or recommended vaccinations are free or low-cost based on eligibility.

What parents MUST bring when coming in for immunizations and/or to get a blue card:

  • Parent Photo ID
  • Child's Social Security Number
  • Child's Immunization/Shot History
  • Insurance Card

If your child is up-to-date on immunizations and only requires a new blue card, please bring the items mentioned above to the Immunization Clinic (251 N. Bayou Street) and allow 48 hours for processing and pick-up.

Walk-ins are accepted only at the MCHD Downtown (Main Office) (251) 690-8821. Immunizations by appointment are available at various Family Health locations.

MCHD and its primary care division — Family Health — are offering immunizations to students at all of its health centers conveniently located throughout Mobile County. For our convenient locations or times, please visit

Patients who receive their vaccinations through Family Health will receive the up-to-date COI at no charge.

All non-Family Health patients will be charged an administration fee of $10 per patient to receive an up-to-date COI. If vaccines are not up-to-date, parents or guardians should follow-up with their primary care provider or schedule a same day/next day appointment at one of the Family Health’s clinics by calling 251-690-8889.

Clients not receiving services through Family Health will be charged a one-time administration fee of $30 per patient if requesting Family Health to enter immunizations into the state system from other states to generate an Alabama COI.

Religious Exemptions

As of July 28, 2017, the religious exemption is no longer the preprinted certificate with the State of Alabama sticker affixed. The current religious exemption form is now similar to the COI document as both are printed directly from ImmPRINT. However, the religious exemptions already distributed prior to the change in format are still valid and do not need to be replaced.

Individuals requesting a religious exemption must submit a Statement of Intent.

Religious exemptions may be obtained by appointment only on the First Floor of the Keeler Memorial Building located at 251 North Bayou Street in Downtown Mobile.
