Family Health awarded two Community Health Center Quality Recognition badges
MOBILE, Ala. — Family Health has been awarded two Community Health Center Quality Recognition (CHQR) badges from the Health Resources & Services Administration (HRSA).
CHQR badges recognize Health Center Program awardees that have made notable achievements in the areas of access, quality, health equity, and health information technology using Uniform Data System (UDS) data from the most recent reporting period. Family Health received its badges for:
Advancing Health Information Technology (HIT) for Quality. This badge recognizes health centers that meet all criteria to optimize HIT services such as: Adopted an electronic health record (EHR) system; Offered telehealth services; Exchanged clinical information online with key providers’ health care settings; Engaged patients through health IT; or Collected data on patient social risk factors;
National Quality Leader for Diabetes Health. This badge recognizes health centers that meet or exceed at least two of the three measure targets for Adult Body Mass Index screening and follow-up plan, Weight assessment and counseling for nutrition and physical activity for children and adolescents, and Diabetes/Hemoglobin A1c Poor Control.
Family Health is one of 17 Federally Qualified Health Centers in Alabama. Of those, only seven received the Diabetes badge.
