WHO WE ARE

Since 1979, Family Health has provided comprehensive primary care services throughout Mobile County. There are now 13 health center sites spanning from Mount Vernon to Tillman’s Corner to Baldwin County. The sites, which are all designated as Federally Qualified Health Centers, are strategically located where the needs are the greatest and access to care is most limited. Family Health is accredited by the Joint Commission and has been designated as a Primary Care Medical Home through the Joint Commission since 2013. Our two mobile health units, which feature two state-of-the-art exam rooms, also bring primary care and wellness screenings to residents within communities who might not otherwise have the means to travel to a health center. The Mobile County Health Department, the parent organization of Family Health, is a Federal Tort Claims Act deemed facility. This health center receives HHS funding and has Federal PHS deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

The mission of Family Oriented Primary Health Care Clinic, Inc. (FOPHCC) is to provide high quality, culturally diverse primary care, preventive health services,  dental care and social services to all those in need regardless of ability to pay.

We are a partnership between the Family Oriented Primary Health Care Governing Council and the Mobile County Health Department, connected by a joint vision of a healthier Mobile County and unified in a mission “to serve the residents of Mobile County by providing high quality comprehensive primary care, regardless of social circumstances or ability to pay, for the improvement and protection of the public’s health.

PATIENT PORTAL

Ask a health-related question, request prescription refills and appointments online via our patient portal. Just register at the front desk for your password and login information and click here to loginThis Web-based Patient Portal gives patients secure and convenient access to their health information. Patients can use the Patient Portal to view, download, and transmit their health information, and send secure messages to their provider. This is not an emergency service; so always call 911 if you have a true emergency.

Vital Records

You may apply for an Alabama vital record at the main Health Department site in downtown Mobile. It’s easy. Fill out an application at the site and give payment and application to the clerk. You may also download an application and fill it out beforehand.

Immunizations

Alabama’s School Immunization Law Section 16-30-4 states each student enrolled in daycare, Head Start and public or private schools have a valid Certificate of Immunization (COI) — previously known as “blue cards” — or a Religious Exemption Form on file at the facility they attend.

School Rules Chapter 420-6-1 (1) states the COI will be on a form approved by the Alabama Department of Public Health (ADPH), which is the one printed by ImmPRINT. All required vaccines must be recorded before the COI can be submitted to the schools.

All required or recommended vaccinations are free or low-cost based on eligibility.
